OVERVIEW:

The LPN is responsible for supervising nursing personnel to deliver nursing care and within the scope of practice coordinates care delivery, which will ensure that patient’s needs are met in accordance with professional standards of practice through physician orders. center policies and procedures, and federal state and local guidelines.

This LPN position is a staff position that has direct care responsibilities as well as supervisor responsibility for nursing assistants.

Responsibilities:

• Receive report at the beginning of the shift; may give report to on-coming shift
• Inform nursing staff of new admission arrivals and the expected time of arrival, as assigned
• Ensure that rooms are ready for the new admission
• Attends shift staff meetings on a regular basis to assist in identifying and correcting concerns and to assist in the improvement of services
• Identify issues or potential issues in the delivery of nursing care to resident and implement corrective action immediately
• Make rounds of unit at beginning of shift and periodically to determine resident’s needs, ascertain quality of care and to monitor the environment for optimum comfort and safety of residents, staff and visitors
• Review the nurse’s notes to ensure that they are informative and descriptive of the nursing care being provided

Mandatory Qualifications:

• NYS LPN Certification
